Palliative and Terminal Illness Program

Navigating complex health and care decisions for disease progression and end-of-life care

Palliative and Terminal Illness Program

Complex Health Decisions

Receiving a terminal illness or end of life diagnosis is devastating for both patients and loved ones. It is tough to make any complex decisions objectively, without the effects of the emotional turmoil of grief clouding one's judgment. There are many clinical and emotional support options to help guide you, with specially trained and educated care support through palliative and hospice services.

In addition, having a health care coaching subscription with a clinically trained and experienced former hospice nurse care manager, allows you to set time aside on your terms on a regular or ad hoc basis to co-creatively work through those complex decisions from diagnosis through the end of life and even beyond for the family.

My role is as a clinical disease process navigator and coach, rather than a therapist or counselor. Areas we can work on:

Adapting lifestyle to clinical and comfort needs
Find resources, education and evidence-based best practice
Exploring alternative medicine approaches
Navigate changing disease progression expectations and changes
